Job Description:
We are seeking a full-time Physical Therapist Case Manager to
provide skilled home health services in Santa Cruz County.
Responsibilities include evaluating patients, developing care
plans, assisting with mobility and independence, coordinating with
physicians, and supervising PTAs.
