FellowshipLIFE is searching for an exceptional part time or per diem Social Worker with NJ LSW who is passionate and understanding of hospice philosophy and principles of death and dying. Interested candidates will have a meaningful impact on the lives of our cherished patients and their families.*Responsibilities:* Plan, coordinate, manage and implement support packages to help patients deal with difficulties during the end-of-life transition. Develop, prepare, and maintain clinical documentation with accuracy, timelines and according to prescribed policies. Monitor and evaluate patients progress and modify care plans accordingly. Advocate for patients and their families and assist them in obtaining appropriate care and access resources aimed at increasing their quality of life. participates with the interdisciplinary team.*Requirements:* Masters degree in social work NJ LSW Understanding of or willingness to learn hospice philosophy and principles of death and dying. 3+ years experience in a healthcare setting, community health experience preferred.
